Position Summary:

The Mobile Crisis Case Manager, in coordination with Mobile Crisis Clinicians facilitates inpatient admissions to hospital, ATU, CSU and Withdrawal Management facilities, and when appropriate, provides on-site administrative support in locations where clients experiencing crisis may be located (i.e., hospital emergency departments, schools, DHS offices, etc.). The Mobile Crisis Case Manager also coordinates transportation for clients requiring admission to treatment facilities.

Essential Duties and Responsibilities:

  • When seeking placement for clients in crisis, the Case Manager collects and organizes all crisis documentation, and transmits to treatment facilities. Works with inpatient facility admissions staff and facilitates communication between mobile crisis clinicians and facility medical providers regarding client disposition.
  • Maintains current listing of all inpatient treatment and withdrawal management facilities, with contact information and admission criteria.
  • In coordination with Mobile Crisis clinicians, provides resource information for clients in crisis needing community services (i.e., housing, child care, medical care, etc.)
  • When needed, provides on-site support in situations where the Mobile Crisis clinician may be working remotely, by assisting with video equipment, managing paperwork, and supporting local community entities in crisis situations.
  • Assists Crisis Services Director with administrative tasks such as creating/updating Crisis Services Handbooks, Mobile Crisis scheduling, coordinating meetings with local community stakeholders, etc.
  • Enters Mobile Crisis data into the Colorado Crisis Hotline web portal.
  • May assist with local transport of clients in crisis.
Requirements:

Education:

Bachelor’s Degree in human services field required.

Certifications, Licenses:

Must hold valid CO Driver’s License.

Experience:

Direct or administrative experience in the behavioral or health care field.

Abilities, Knowledge, Skills:

  • Computing skills, ability to manage detail, strong communication skills, some knowledge of behavioral health and health care coordination.
  • Ability to travel within the Center’s service area.
  • Ability to work 24-hour shifts.
  • Demonstrates positive interactions with others
  • Displays enthusiasm toward work and the mission of the organization
  • Demonstrates professional image, maturity, and positive attitude
  • Genuine interest in Centennials mission and a willingness to work with vulnerable populations utilizing compassion and empathy in decision making and customer service
